Forma-A for Medical Leave to be Submitted by the Employee/Teacher who wants to undergo on Medical Leave. Recently District Educational Officer, Ranga Reddy (DEORR,RRDEO) has ordered DDOs (HMs/MEOs) to optain Form-A from their Sub Ordinates while going on Medical Leave and also MEOs and Dy.DEOs have to submit the same form to DEO,RR.
RRDEO has stated that it is essential. Here the Form-A to be issued by the Doctor reffering rest to the concern employee
